The St. John Kidney Transplant Program accepts referrals from three primary sources: nephrologists, dialysis centers and self-referrals. St. John Kidney Transplant Program Xanthus Building 1923 E. 21st Street, Suite 101 Tulsa, OK 74114-1430 o Within one business day from receiving the referral, the patient and referring person will be contacted by a transplant coordinator for a pre-screening interview. o If the patient meets requirements following the pre-screening interview, he/she will receive a call from a financial counselor to start an insurance review. o Qualified patients will have an initial evaluation within 30 days of referral. |
