Home health care is a short-term nursing and skilled care service to help you adjust to home from the hospital or to help you with other needs at home.

St. John Home Health is state-licensed, Joint Commission on Accreditation of Healthcare Organizations (JCAHO) and Medicare certified, and a member of the Oklahoma Association of Home Care.
Who Needs Home Health?
Patients with physical limitations that do not allow them to adequately care for themselves once they leave the hospital. Home health addresses a number of concerns, from personal hygiene to ostomy and tracheotomy management.
A physician will evaluate your situation and if appropriate, refer you for St. John Home Health for continued, short-term nursing care once you leave the hospital.

Regular office/services hours are 8 a.m. to 5 p.m. Monday through Friday, but weekend and holiday service is provided based on the patient’s plan of care and condition. After-hours triage is provided by an on-call nurse and is available by calling the office at 918-747-7901.
St. John Home Health can perform many treatments and procedures in the home, using a variety of disciplines. Your physician will determine which home care services are appropriate for you.
The St. John Home Health team includes certified team members in skilled nursing, nursing assistants, physical therapists, occupational therapists, speech therapists, and medical social services.
Nurses perform prescribed treatments and procedures while supervising and instructing the patient and family. Specialty services may include, but are not limited to:
• IV therapy
• Ostomy management
• Wound management
• Diabetes management
• Tracheotomy management
• Enteral nutrition
• Ventilator-assisted breathing
• Medication management and teaching
• Cardiac care program
• Nutrition assessment by a registered dietitian

Home Health Aide
Home health aides work under the supervision of a registered nurse or therapist to assist with personal hygiene and other non-medical services. Typical duties include:
• Taking vital signs
• Bathing, shampoo, and other necessary personal care recommended by the home care staff
• Helping the patient out of bed and to walk around the house
• Light meal preparation
Physical therapists evaluate the patient’s condition and provides prescribed treatment to improve the patient’s physical condition and increase independence.
Using exercise, self-help devices and activities, occupational therapists help you become more self-sufficient in feeding, dressing, personal hygiene, and homemaking skills.
Speech therapists help you regain ability to communicate while counseling on therapy application and communication disorders. Speech therapists also provide evaluation and treatment of swallowing and chewing difficulties.
Medical Social Services
Staff members assist you and your family in dealing with social, financial, and emotional factors relating to our condition; help you get in touch with hospital, government, or community services; and make referrals for lab work, medical supplies, and equipment.

Referrals to St. John Home Health may come directly from family members, friends, healthcare professionals, or the patients themselves. Each case is assessed by the patient’s physician to determine appropriateness for home health care.
The cost of home health care is covered by Medicare, most health insurance programs, various government programs, workers’ compensation, and the Veterans’ Administration, or may be paid privately by the patient.
Medicare pays for home health staff services (except intravenous [IV] medications) with no cost to you unless you receive written notification from Medicare that your care is not covered. Should this occur, our staff will notify you, discuss your care needs, and obtain written permission from you to bill you directly if you wish care to be continued.
Other Insurance Coverage
If your coverage is through health insurance other than Medicare, the intake/insurance reimbursement nurse will contact the insurance company to verify benefits and obtain preauthorization, if required. You will be given a written estimate of any charges.
